Social Services

The end of life journey can be a difficult time with many unknowns. The goal of social work is to provide non-medical services and resources to further a patient’s ability to stay safe in their home and reduce re-hospitalization. Every patient under our hospice care is assigned a social worker whose job it is to help our patients and their loved ones navigate these challenging areas. Our social worker collaborates with the nurses and therapists to ensure coordinated care. Social work is not just for the patient but is guided under a holistic approach involving family members, Powers of Attorney, caregivers, and other community case workers.

Life Changing Services

Hospice is not just for people with cancer. Patients suffering recent deterioration, frequent emergency room visits and hospitalizations or suspending aggressive treatment may also be eligible for hospice care. Hospice care can be discontinued by the patient at any time.
